Billing & Insurance

It is your responsibility to become familiar with your health insurance policy, particularly the limitations and exclusions of that policy. If you have any questions regarding your policy, we urge you to contact your carrier directly. Please be prepared to provide complete insurance information at the time of your visit. A photocopy of your identification card will be made and placed with your records. Depending upon your coverage, you may be personally responsible for payment of a portion of the charges incurred for services provided during your visit. For those patients who have a financial hardship, our Billing Coordinator will give you the necessary form for assistance or will work out a payment plan for you. If your insurance requires a co-pay, it is expected at the time of the appointment.
